The total cost for camp tuition, room, and board is $550. A deposit of $250 is due by May 1 at the latest—however, it is helpful if you send your deposit as soon as possible; the balance of $300 will be due when you check in at camp. Please indicate on this form if you are receiving a scholarship from your local jazz society. Some scholarship funds are also available from Camp Heebie Jeebies on a first-come, first served basis.
The final concert is Friday, July 6, beginning at 6:30 p.m. If your parents can attend the concert, you may leave the camp with them (or another authorized person). If you do not leave on Friday night, the dismissal time is 9:00 a.m. on Saturday, July 7. Please let me know if you need any help with travel arrangements.

♪ Driving? Arrive at Camp David Jr. around 4:00 p.m. on 7/3. Take Highway 101 west of Port Angeles 30 miles, turn right on Camp David Jr. Road. Parents wishing to volunteer as cabin counselors--please call 406-862-3814 for details.
